Kenwood K-8 Center PTSA Crest Order Form Kenwood Crests are available for $3.00 each. Please fill out this form completely, enclose either cash or a check made payable to Kenwood K-8 PTSA. If you are paying by check, please print student’s name and homeroom teacher on the memo portion of the check. You may give the order form and payment to your child’s teacher or you may turn them in to Kenwood’s office. Crests will be delivered to your child via their homeroom teacher.
